EKU Online DNP Capstone Project: Evaluating the Effectiveness of a Heart Improvement Toolkit Program, Cheryl Mitchell
Cheryl Mitchell, an EKU Online DNP student, completed her degree by presenting capstone project "Evaluating the Effectiveness of a Heart Improvement Toolkit Program: A Pilot Project" on Friday, May 1, 2015.
The purpose of theproject was to implement the Heart Improvement Toolkit (HIT) Program for patients with a heart failure diagnosis who have been admitted to non-cardiac specialty units. The project was implemented at UK Chandler Hospital in Lexington, Kentucky. Patients received one-on-one, comprehensive, heart failure-specific education delivered by a nurse in advanced practice. Program evaluation included 15- and 30-day readmission rates, the occurrence of adverse clinical events, and patients’ self-care behaviors related to heart failure management.
Members of the research committee were Dr. Mary DeLetter (chair), Dr. Ida Slusher and Dr. Cathie Velotta.
